Form 1583: Application for Delivery of Mail Through Agent
The USPS Form 1583 (also referred to as Application for Delivery of Mail Through Agent) is a mandatory authorization form required by the United States Postal Service whenever a customer wants a third party (like Bizee or any commercial mail receiving agency) to provide a Virtual Address and receive and manage mail on their behalf.
Bizee will facilitate this process through a third party for you to be able to verify the document through your Dashboard.
Note: Your order needs to be completed for you to be able to complete the verification.
Verification Steps and Accepted Documents
To activate your Virtual Address service, you will need to upload one valid photo ID and one document that verifies your current address. This is a standard USPS requirement that applies to all customers using a virtual address in the U.S., and unfortunately, we cannot make exceptions—only officially accepted documents will be approved.

Important Notes:
- The two documents must be different—you can’t use the same one for both steps.
- Expired or unsupported documents (e.g., bank statements, screenshots, SSN, application forms, or utility bills) will be rejected.
- Both documents must be clear and readable in full.

Step 1: A Valid Photo ID
This must be a government-issued ID showing your full name and photo. Accepted options include:
- Passport
- U.S. Driver’s License
- U.S. State ID
- U.S. Permanent Resident Card
Note: For international customers, only a passport is accepted for this step. Other foreign IDs are not permitted by USPS.
Step 2: A Document That Shows Your Address
This must show your full name and physical address, matching the information you submit on the application. Accepted documents include:
- Voter Card or National ID
- Vehicle Registration
- Current Residential Lease
- Home or Auto Insurance Policy
- Mortgage or Deed of Trust
- United States (only) Driver’s License

Fill out your Photo ID Verification information. This must match the document you will upload later.
- Ensure the Document Identification Number matches the selected document, for example, Passport number or Driver's License number, that you will upload.

Please provide all fields. The issuing entity is the government institution that issued the document. Include the expiration date and your date of birth.

For Proof of Address Verification, verify the information on the document you will upload.
- The document number should match the document you will upload, for example, the Mortgage Agreement Number or Home/Vehicle Insurance Policy number.
- If the file you are uploading does not have a document number, you can type in five zeros (00000).

Ensure the address is as close as possible to the one listed on the Document.

Confirm the information and the button to "Start the Verification Process."

A pop-up will appear. If continuing on the same device, click "Continue on this device." If using a phone, scan the QR code to continue.

On the next screen, verify the name and address. If correct, click "Start process".

Review the photo requirements for the Proof of Identity on the next screen. Then click "Continue."

Select the document to upload. The document you need to provide will be listed at the top, for example, the passport. Click "Browse Files" to upload, or click "Take a photo with your camera" if using a phone.

Once the document has been selected, you will be given a small preview. If it looks good, click "Continue."

Review the photo requirements for the Proof of Address on the next screen. Then click "Continue."

Select the document to upload. The document you need to provide will be listed at the top. For example, the US State ID Card. Click "Browse Files" to upload, or click "Take a photo with your camera" if using a phone.

Once the document has been selected, you will be given a small preview. If it looks good, click "Continue."

You will be asked if the information can be found on the provided document. If it is included, just click "Submit" to upload the documents.

Once the documents have been uploaded, you will be given a message that the upload was Submitted. We will review the information, which usually takes a business day.
- Once it is confirmed that the uploaded documents match the information provided, a new action item will appear on your dashboard, requiring your signature on the USPS Form 1583. Once the Form is signed, the process will be complete.
